In consideration of my role with Calvary Chapel, its affiliates, integrated auxiliaries, and supporting organizations, hereinafter “Calvary Chapel McKinney” and recognizing the importance of my character being above reproach, I HEREBY CONSENT TO THE FOLLOWING:

• ︎ Complete in full a Calvary Chapel McKinney Background Information Form.
• ︎ Allowing Calvary Chapel McKinney to screen me through the Texas Department of Criminal Justice of any other criminal history records as well as any other historical or background records pertaining to me.
• ︎ Allowing Calvary Chapel McKinney to verify the information and to contact any personal references listed on the Background Information Form.
